In each of our six clinic locations, we offer our prescription assistance program to patients who cannot afford to pay the high cost of certain medications. Eligibility:
- No prescription coverage or fall in the Medicare coverage gap
- Meet the Pharmaceutical company’s individual income guidelines
What you get:
- 3 month supply of the ordered medication
What you need:
- $5.00 per medication
- Your latest income tax filed, if you filed; SSA Statement; Paycheck Stubs; Unemployment Statement or any other proof of household income
